With more than eight years of clinical experience, including practice in multiple Level I trauma centers, Faith is experienced in caring for patients with complex medical conditions and specialized nutrition needs.

What Referring Providers Can Expect

  • Evidence-Based Care: Nutrition recommendations are grounded in current evidence and individualized to the patient's medical history, treatment plan, nutritional needs, and goals.

  • Collaborative Care: Nutrition care is intended to complement—not replace—medical treatment. Communication with the referring provider is available when clinically appropriate and with patient authorization.

  • Practical Recommendations: Patients receive realistic, actionable nutrition strategies designed to fit their medical needs and daily lives.

  • Ongoing Communication: With appropriate patient consent, relevant updates and care information can be shared with the referring healthcare team.

Referral Process

1. Submit a Referral

Please email Faith Mather Nutrition Care with the patient's name, contact information, and reason for referral. If the patient's insurance plan requires a physician referral or order for nutrition services, the office will contact you or the patient to obtain the required documentation

2. Patient Scheduling

Faith's office will contact the patient to coordinate their initial appointment and review insurance or private-pay options.

3. Nutrition Assessment & Care

The patient receives an individualized nutrition assessment and treatment plan based on their clinical needs.

4. Ongoing Collaboration

With patient authorization, relevant updates can be communicated back to the referring provider.


Accepted Insurance

Faith Mather, RD is currently in-network with Aetna and UnitedHealthcare. Additional insurance plans are being added as credentialing is completed.

Coverage varies by individual plan and diagnosis. Benefits will be verified prior to the initial visit whenever possible.
